Use the form on this page to create a list of specialties/procedures that can be imported into the app rather than using the app to add procedures. Submitting the form will create a file that is downloaded to your computer. The file must then be transferred to the Procedure Log app and imported into the list of procedures available in the app by following the instructions given in the form's page. The file can also be shared with other users for them to transfer and import into the app.
