• Diagnoses can now be selected from a tabbed view controller containing two tabs: Favorites and ICD-10 Dx.

• Table of ICD-10 diagnoses loads faster.

• Added ability to create favorite diagnoses from ICD-10 diagnoses (swipe left on diagnosis in the ICD-10 view and tap "Favorite" button); diagnosis will then be shown in the Favorites list.

• Removed "Edit Diagnoses" from Settings view, since they can be edited from within the diagnoses lists (see Help/Instructions in the Info section).

• Added ability to add additional images/photos without having to tap the camera icon again; just tap "Save and add another" when saving image.

• Improved the speed of creating reports by not creating a separate Zip file of the images. Images/photos and signature image files are still included in the file.

• Completely rewrote the app to use an SQLite database to store all records.  The first login after updating will transfer all your records and other data to the new database and create a backup of the old data in CSV files (saved in a password protected file).

• Updated code to require minimum of iOS version 8.0 to run the app.

• Added a "Complications" field to record any complications that occurred from the procedure.

• Added ability to capture images from the camera and the photos library.

• Added option to use ICD-10-CM diagnosis codes.

• Added ability to select more than one diagnosis for a given procedure record.

• Added "2nd Assist" and "Observed" as additional options in the Performed/Assisted field.

• Updated Privacy Policy to clarify that entering one's Date of Birth and Social Security Number are optional, and, if entered, are stored in a secure manner only within the iOS device and are not sent to the app's developer or any other third party. They are used only for verification if one forgets one's username or password.

• You can now save a copy of your reports and Backup folder to DropBox (requires a DropBox account and the DropBox app on your iOS device.

• Fixed an issue where selecting "Assisted" in the Add to Log view would still list "Performed" in the log.  One can now change whether a procedure in a previous entry was Assisted or Performed by tapping on the entry in the Procedure Log view, then tap on the Assisted or Performed row.  However, this will not change the text in signature attestations that were previously signed by your supervisor; they will still say "perform."

• Fixed an issue in iOS 9 where the confirmation notice "The entry was saved to the Procedure Log" was not displaying after saving a new entry.

• The Version History, Help/Instructions, and Privacy Policy views now start from the top on iPhone 6, 6s, 6 Plus and 6s Plus.

• Fixed an issue when scrolling to a section from the Table of Contents in the Help/Instructions view.

• Minor update to the Detail View section in the Help/Instructions view noting that "Assisted or Performed" can now be changed.

• Fixed an issue where the procedures supervised listed in the signature attestation, under some circumstances, were not correct when obtaining a supervisor's signature from the "Details" view.

• Retrieve your username and password if you have forgotten them; you must first answer some security questions in the "Create User" or the "Edit User" views.

• If you delete the app from your iPhone, then reinstall it, you are now given the option to delete the items stored in the keychain—your name, institution name, email address, username and passwords, both for the app itself and for your reports. You can now re-create a new account. NOTE that if you delete the app, all your logged data is deleted with the app.

• Minor fix in the line spacing of PDF reports if notes are included but no signature attestations.

• Reports can now be created for a specific date range.

• Added the ability to specify the primary, secondary and tertiary sort order of the PDF report.

• Removed the "Reports" section from the "Settings" view and moved it to the "Create Reports" view, which is accessed by tapping "Reports" in the main "Procedure Log" view. From there, select the date range for your reports, the sort order of the detailed items in your PDF reports, and set the other report settings.

• If an entry is saved without the supervisor's name or supervisor's signature, one can now add the supervisor's name and the signature at a later time.

• If Touch ID is set to ON, it will be activated automatically when the app loads (no need to touch a text field first). Other minor improvements to Touch ID handling.

• UI improvements to the "Select Procedures" and "Edit Procedures" views.

• Added "Version History" view to "Info" section.

• Fixed an issue with deleting signature image files when the logged entry containing them is deleted.

• Minor UI changes and fixes to "Add to Log" view.
